Clean Cyst Removal

Sebaceous Cyst Removal

In this video, Dr. Nakatsui removes a sebaceous cyst from our patient’s scalp. The cyst came out whole, cleanly, and intact! Many times, if a cyst becomes infected it can adhere to the surrounding tissue making extraction trickier as it can fragment and stick to surrounding tissue.

Overall a great little surgery which great end results!

In this video Dr. Nakatsui is removing two sebaceous cysts located on the scalp. Our lovely patient graciously allowed us to film the procedure and we thank you for that!

In this video the two cysts come out relatively easily and intact as a whole. There will be several instances where we are not so lucky and the cyst may fragment or it may adhere to surrounding tissue more making extraction more difficult.

Sebaceous cysts may occur anywhere on the body at any time of our lives. They are generally not life-threatening, but when concerned about any lesion, it is always best to seek medical attention. Many patients with sebaceous cysts may leave them as they generally will not bother them. From time to time, cysts may become inflamed, painful, produce discharge, or become infected. If a cyst has had repeated infection/inflammation or self attempts to extract the contents by squeezing the cyst, this is when the cyst wall may become more adherent to surrounding tissue making it difficult to extract.

Should a cyst develop, it is advisable not to perform self surgery as any tools used to puncture the skin or extract the cyst may push unwanted bacteria into the site causing an infection. Not only that, if the wound site is not properly closed or should become infected, when it fully heals you may be left with an unwanted scar which may be difficult to treat in the future.

Blood Vessel (Telangiectasia) Treatments

Broken blood vessels, or telangiectasias, are a very common and typically non-threatening condition that can affect any part of the body. Telangiectasias can naturally arise at any time, however, individuals that have chronically exposed their skin to the sun or extreme temperatures may be more susceptible to telangiectasias. 

For the majority of times, once a telangiectasia appears, it rarely if ever goes away on it’s own. Over time people may notice an increase in these broken blood vessels. Usually most people do not care if the telangiectasia appears on their arms, legs, or torso as it can be easily covered; however, once they become more apparent on the face and neck, then it becomes more of a cosmetic concern.

The typical treatment for telangiectasias at our centre involves the use of a laser called the V-Beam pulsed dye laser. This laser uses a specific wave length of light to destroy the hemoglobin in the red blood cell, which is predominantly what is seen in telangiectasia. The energy is absorbed by the hemoglobin and bursts the cell. Once the treatment is complete, the body reabsorbs the vessel. Because the laser is physically bursting the vessel, a bruise may become evident which can last for approximately 5-10 days. 

The treatment is fairly quick and has relatively low pain associated with it. Most of our patients say that it is similar to a rubber band being snapped onto your skin. It is advisable that should you go for a V-beam pulsed dye laser treatment (or all lasers for that fact), it is important to do your best to avoid excessive sun exposure before and after the treatment for as long as possible.

In this video, we are seeing what Dr. Nakatsui sees when performing surgery. This surgery is the removal of a lipoma, a benign fatty growth that can occur anywhere on the body. Many are small, but lipomas can grow very large in size. Dr. Nakatsui makes a small excision where the lipoma is located. This particular lipoma came out relatively easily, but keep in mind that not all lipomas will be this easily extracted. To close the surgery site, Dr. Nakatsui uses a subcuticular running stitch.

Let’s talk about scars

Scar treatmentScar treatment can be challenging and requires a multi-dimensional approach in order to be successful. A scar can be the centre of attention but can also be a source of annoyance. For some individuals, scars are a part of who they are, whereas for others they are intrusive and cause insecurity. They can be caused by a variety of reasons such as acne scarring, trauma, and surgery.

Some individuals have thickened keloid scarring, where a scar becomes raised and discoloured. In order to treat the thickened aspect of the scar, many times cortisone injections will be utilized to reduce the thickness of the scar.

On the opposite end of thickened scars, people may get pitted scars where the skin sinks in various regions. These types of pitted scars can be tricky to treat; however, with improvement in technologies and techniques, pitted scars can often be improved. In some cases, Dr. Nakatsui will use an ablative laser to smooth out the skin and reduce the harsh edges that cause shadowing.

A new approach is to promote collagen re-growth in the region.  Through the use of a technology called Percutaneous Collagen Induction (PCI), many tiny needles create tiny holes in the skin. The creation of these tiny holes stimulates collagen formation to help fill in the areas of depressed scarring.

Scar treatment also encompasses hyperpigmentation (darker pigmentation), hypopigmentation (lighter pigmentation), and erythema (redness).  Hyperpigmentation and erythema can be treated fairly successfully with various lasers. Unfortunately, hypopigmented scars are more difficult to treat and currently there is no reliable solution to permanently bring colour back into a scar once the colour is gone.

Certain scars due to trauma are very visible to the human eye because the lines of the scar are fairly linear. Surgically removing the scar and carefully using various closure techniques to break up the linear scar can greatly reduce the appearance of a scar.

Scarring can impact an individual’s self-confidence. With current technology, scars can be improved. For those with scars that affect their self esteem and their daily living, don’t get discouraged as there are many great options to help treat your scars.

May is Melanoma Awareness Month

Did you know that in 2017, approximately 7,200 Canadians were diagnosed with Melanoma?  Melanoma is the most dangerous form of skin cancer that has been on the rise in Canada and across the world.  If melanoma is diagnosed early, there is a high probability of successful treatment and curability!  Staying vigilant and examining all lesions on your body regularly can help you stay on top of our skin health.

Here is a case example of melanoma with Dr. Nakatsui.

Mole

One of Dr. Nakatsui’s patients presented this mole to him and when presented this mole (above). Dr. Nakatsui had a strong suspicion that this oddly shaped mole could be something sinister. Through a diagnostic biopsy performed by Dr. Nakatsui at the Groot DermaSurgery Centre, the results confirmed that the mole was indeed a malignant melanoma, the most worrisome form of skin cancer.

What made Dr. Nakatsui concerned about the mole? We can look at the ABCDE’s of mole checking.

AAsymmetry: The mole is asymmetrical in shape. If you draw a line through the middle, it is not perfectly symmetrical.

BBorder: The border is uneven and ill-defined. Malignant melanomas typically have uneven, scalloped, or notched borders.

CColour: The colour variation in this mole was subtle; however there are elements of light and dark brown, and even some shades of dark blue. Having a multitude of colours in a single mole can be a warning signal. These colours can be reds, browns, blues, whites, and/or blacks.

DDiameter: The physical size of the mole was another concern for Dr. Nakatsui. The mole was approximately 2.0cm x 1.6cm in size. Malignant melanomas usually are larger than a pencil eras in diameter (6mm), but many malignant melanomas can be smaller than 6mm in size when they are first detected.

EEvolution: This patient noted that the mole had changed in a relatively short period of time. It grew in size rapidly and it changed in colours within a few months. Any change in colour, shape, elevation, or any other trait, or even new symptoms such as crusting, bleeding, or itching should be examined by your family doctor or dermatologist.

It can be difficult to determine whether a mole is good or bad and the best way to be certain is to have your family doctor or dermatologist examine your moles.

Keep tabs on your moles – Take monthly photos of any or all moles. Use a ruler and place it beside the mole and take a photo. A couple weeks to a month later, take another photo with the ruler again and determine if there is any change to the mole in size, shape, colour, etc. If you do note changes, it is time to see your doctor.

Avoid sun exposure! Wear hats, sunscreen, long shirts to cover your arms, and long pants to cover your legs. Sun exposure can increase the chances of malignant melanomas and also increase the risk of other forms of skin cancers such as basal cell carcinomas and squamous cell carcinomas.

There is a genetic component as well.  Ask your immediate and extended family members if they have had melanoma.

Let’s talk about it!  Mental health, let’s make it loud and clear!  Mental health is nothing to be ashamed about.  Anxiety, stress, depression, negative thoughts, and contemplation of suicide are very real and very serious issues.  Over 250,000 Canadians will suffer from some form of depression in their life time.

Depression can be silent.  Much like our shadow following us wherever we go, sometimes the shadow of depression grows larger and darker.  Please know:

What can be done about beards?

In today’s society having a full and lush beard is becoming more and more of a fashion statement for men.  While some men are born with thick facial manes, others require treatment to aid in thickening up their beards.

Not all men are lucky like actor Kristofer Hivju (photo), more popularly known as Tormund from the HBO hit show, Game of Thrones (most of the staff at the Groot DermaSurgery Centre are very big fans of Game of Thrones).

Dr. Thomas Nakatsui, a board certified Dermatologist and hair transplant specialists, has been performing hair transplants for over 18 years.  The exact reasons why men come in for a beard transplant vary.

Some men want to increase thickness, while some men have suffered some form of trauma that caused an area to scar, therefore leaving a portion of facial hair missing.

As with any hair transplant, artistry plays a major role in how the end result turns out.  Hair transplants need to look natural and undetectable in appearance.  Old forms of hair transplants, “plug transplants”, have yielded unnatural and very detectable results.

Having undetectable results is even more paramount whenever dealing with the face as it is always front and center.  Any irregularities can be easily spotted.

To address this issue, Dr. Nakatsui follows the patient’s natural flow, angle, and direction of their beard.  If hairs are placed in any strange angle, it will become very apparent that a hair transplant was performed once the hairs have grown in.

The size of the graft is important as well.  A graft is a natural grouping of hairs that come in 1, 2, or 3 hair bundles.  Dr. Nakatsui typically utilizes the grafts with one hair when it comes to beard hairs.

Beard hair transplants can be a great tool to help thicken up a beard.  One thing to consider before having a beard hair transplant is that the donor hair, which is usually taken from the back of the scalp, may have different texture and/or colour than the recipient bearded area.

Once the transplanted hairs are placed, it will hold the same characteristics as the area where it was taken from.  If there is a significant difference between the donor hair at the back of your scalp and your beard hair, it can potentially become visible and unnatural.

The beautiful thing with a beard hair transplant if it is done correctly is that it now gives the individual the choice whether or not they want to grow a beard or be clean shaven.

Because of the incredibly small incisions that Dr. Nakatsui makes during the procedure, a patient can be cleanly shaven and have no trace of scarring to the area.

Pumpkin spice everything, leaves falling, and sweaters everyday can only mean one thing:  Fall is here!  This beautiful and romantic time of year can also mean changes in weather and temperature which can affect your skin.  Everyday events that we take for granted, such as stronger winds, can strip away moisture from the skin.  When this moisture is stripped or there are irritants in the air, your skin can become itchy or even a rash can appear!  Here are some basic tips to help your skin retain moisture.  The following are only tips and does not replace seeing a qualified dermatologist.

  1. Shorten the time spent in showers and baths.

Not only is it good for the environment to decrease the amount of water used, limiting the time spent in the shower will prevent your skin from losing its moisture barrier.

Closing the bathroom door to keep moisture in the bathroom is a great way to keep your skin moist.

Hot water can irritate the skin.  Try dropping the temperature to a warm setting.

Use unscented soap products on your skin.

Gently pat your skin dry with a towel.

Immediately after drying your skin, apply a non-irritating moisturizer to your skin.

  1. Apply moisturizers after wash.

Washing your hands will strip away moisture from your skin.  Many individuals, especially those in the healthcare field, wash their hands several times a day.  After washing your hands, lightly pat dry your hands and then immediately apply moisturizer.  This will help keep moisture in your hands.

  1. Wearing gloves.

Wearing gloves when outside is not only a fashion statement, it can help protect your hands from the elements!  Gloves can also help retain moisture on your skin.

Gloves also act as a physical barrier between your skin and other substances in your environment, such as chemicals, potential allergens, and objects than can cause little cuts.

  1. Choose the right detergent for you.

Find a non-irritating laundry detergent that works for you.  It will take some trial and error to find the perfect fit.  Try using an unscented/hypoallergenic detergent to avoid irritation.

  1. Limit time directly in front of the fireplace.

Cozying up next to a fireplace on a chilly autumn day can truly warm the soul.  Just remember fire or any heat source can dry your skin.  Try limiting your time spent directly beside any heat source.  Also, don’t forget that if you are using a fireplace to have a window or door open to allow fresh air to circulate within the home!

  1. Humidifiers

Humidifiers are great tools to help bring back moisture into the air and decrease the chances of your skin drying out.

  1. Don’t forget the sunscreen

Yes, even with the limited hours of sun and increased cloudiness, wearing sunscreen on open portions of skin is still very important throughout the year!  Many sunscreens will also provide a moisture barrier to help retain that needed moisture.

The autumn season is an incredible time of year.  Don’t let dry skin bring an end to your fun.  If you find that your are experiencing prolonged periods of dryness or even a rash, consult with your Dermatologist!

 

Removing The Regretted Tattoo

We’ve heard it all before: a woman falls deeply in love with her boyfriend, Kevin, and decides to have his name tattooed across her arm only to become heartbroken the following week.  She is now stuck with this unwanted tattoo across her arm.

Years ago, she only had a few options: live with the regret and have his name forever inked on her skin, or try to cover it up with a second tattoo on top of her current tattoo.  Various creams have been suggested; however, many of them were either ineffective or, worse yet, caused scarring due to the strong nature of the creams.

The newest and most advanced technique for removing unwanted tattoos is with laser therapy.  At the Groot DermaSurgery Centre, we use the Q-switch Alexandrite laser.  Various lasers are used for various conditions such as excessive hair, mole removal, or unwanted blood vessels.  Each one of these conditions will require a different wavelength light.

Specific to tattoos, the laser penetrates the skin at a specific wavelength with the goal of breaking pigmented particles.  Your body’s immune system then removes these pigmented particles over time.  After a few treatments, the tattoo can potentially be completely erased from a person’s skin.

As with any procedure, especially those that involve lasers, it is always important to be aware of the risks and precautions surrounding the treatment.  Regarding laser therapies for tattoo removal, one potential risk is hypopigmentation, or the loss of colour, of the skin.   Or conversely, a person may get hyperpigmentation, or the increase in pigmentation in the skin.  The laser used in tattoos cannot distinguish between naturally dark pigmented skin and a tattoo, therefore, when the tattoo is being removed, surrounding skin can potentially be affected as well.

At the Groot DermaSurgery when consulting an individual about tattoo removal, we highly suggest that the individual not have any tan in their skin before and after treatment for these exact reasons. As always, speak to a certified medical professional when dealing with any treatment. When it comes to Fillers and Botox®, it is important that you know what the product is and who is doing the treatment and to be wary of discount botox or discount filler injections as the long term results can be more costly. Telogen Effluvium

(Scalp Hair Thinning Due to a Shift in the Hair Cycle)

The sudden loss of exaggerated amounts of scalp hair is quite a common complaint, particularly in women.  This shift in the hair cycle, creating a hair loss pattern called “telogen effluvium”, usually occurs suddenly.  This is not associated with isolated bald patches of the scalp, or with excessive crusting or infections of the hair follicles.  It may “unmask” male/female pattern hair loss, revealing thinning on the top of the scalp.  In these cases, treatment is required for both problems.

NORMAL HAIR CYCLE

When a scalp hair begins to grow in each individual hair follicle, it remains in the “growth phase” (anagen phase) for approximately three years.  This average growth phase for the scalp hair creates hair that will grow to approximately mid-back level.  Some people have a longer growth phase and can grow their hair until they can almost stand on it, while others with a shorter cycle, can grow their hair no longer than shoulder length.  This is an individual genetic variation.

After the growth phase, the individual hair follicles, independent of each other, convert into a “resting phase” (telogen phase), and remain in this dormant stage for approximately three months.  During this time, a new hair is suddenly stimulated to grow, which eventually “pushes out” the old hairs as you comb, brush, and wash your hair.  You will see these hairs coming out all of the time.  This resting hair has the typical white tip on the scalp-end of the hair.  This white tip is not the root, but indicates that this hair was in the resting phase, and that another hair is replacing it in the same follicle.  In the normal hair cycle, approximately 85% of hairs are in the growth phase at any one time and 15% are in the resting phase.  With this ration, it has been calculated that the average person will lose 100 hairs per day.  If you do not shampoo every day, you will notice that on the days you do shampoo, you will lose an exaggerated number of hairs.  This is normal.

TELOGEN EFFLUVIUM MYTHS:

The hair cycle and the growth phase of your hair have nothing to do with having a tight scalp or a lack of blood flow to the scalp.  The tightness of your scalp is genetically determined, and has nothing to do with “circulation”.

Telogen effluvium is not associated with frequent shampooing.  In fact, the healthiest scalps and hair that I see are the patients who shampoo daily.

The length of hair does not affect the growth rate.

WHAT CAUSES TELOGEN EFFLUVIUM?

It is important to remember that after the triggering event that causes a shift in the hair cycle, it takes up to three months before the hair loss is noticed.  Due to this delay, it is important to look back at least three months prior to the onset of your hair loss.  Following are some of the common causes of telogen effluvium:

Pregnancy and Delivery – 8-12 weeks after the delivery of a child a scalp hair loss often occurs that can be quite frightening.  This is temporary, and the hair cycle will re-establish itself.

A high fever (from any cause) is a common initiator of a shift in the hair cycle from the growth phase to the resting phase, the growth phase will return to normal.

Ingestion of various anticoagulants (blood thinners) can affect the hair cycle.

Birth control pills can initiate a shift in the hair cycle, either while the patient is on them or once the patient has stopped the pills.  The same type of hair loss can be experienced as with the birth of a child.

Emotions – some patients have a very sensitive hair cycle and emotionally disturbing experiences can shift the hair cycle, creating an exaggerated hair loss.

Anemia, particularly iron deficiency anemia, can influence the hair cycle in a negative way.

Any severe, systemic illness can create a shift in the hair cycle.

Hypo- or hyperthyroidism can create telogen effluvium.

Diets – any crash diet program can create a shift in the hair cycle (the HCG weight control program often influences the hair cycle).

THERAPY:

Correct the precipitating cause if it is ongoing.

Avoid aggressive combing, brushing, and back-combing of your hair.

Do not “over use” the hair dryer.

Use mild shampoos

It is important to have serum iron and iron binding capacity laboratory tests done, as there are significant number of patients who have low tissue iron levels who will not regain the growth phase of the hair without iron replacement.

It is very important to remember that once the hair cycle has shifted, it takes months for the hair cycle to re-establish itself, even in patients where the cause of the hair loss is easily determined and eliminated.  Few patients never quite regain the volume of hair they had before the shift occurred. If you feel that your hair loss has gone past the preventative stage, you may need a hair loss treatment that is more intensive and more personalized to your particular issue. Thankfully there are plenty of options available in today’s market.

Zinc (Usually in the form of Stress Tabs with zinc and copper, 1 pill twice daily) and Rogaine lotion (1 ml twice daily to thin hair areas, is often helpful but may require many months of therapy).
