COVID-19 Vaccination of Patients on Systemic Therapies

January 27th, 2021 | General Info, Treatments

covid-19 vaccine

A common question we get is whether or not patients on systemic therapies for skin conditions such as psoriasis, eczema, or lupus can receive any of the COVID-19 vaccines. These patients are usually concerned about the possibility that they will be susceptible to developing side effects or infection as a result of the immunosuppressive properties of many of their systemic medications (e.g. biologic medications).

Our advice has been for all of our patients to get vaccinated when they can as long as they do not have a known allergy to any portion of the vaccine. The COVID-19 Pfizer and Moderna vaccines are mRNA vaccines and thus do not contain any live virus. As a result, receiving these vaccines should not pose any increased risk.

Another question we get is whether or not they should discontinue or hold their medication doses around the time of their vaccination to prevent the medication from decreasing their body’s ability to respond to the vaccine. At this point, we believe they should continue their medications when they receive their vaccinations.

The Canadian Dermatology Association recently published a position statement on this and declared:

1. All patients, regardless of ongoing treatment, should receive [COVID-19] vaccination unless they have documented, severe reactions to one or more vaccine constituents.

2. Modest attenuation of vaccine effectiveness is expected with some drugs [but] only when maximization of vaccine effectiveness is of paramount concern should interruption of systemic therapy be considered.

Topical minocycline for rosacea

December 11th, 2020 | Treatments

We have used oral minocycline for rosacea for decades. However, there are risks associated with oral minocycline, even though they are rare. A topical formulation of minocycline is being investigated for the management of rosacea by a company called Vyne Therapeutics. Two studies with a total of 1521 patients compared 1.5% topical minocycline foam versus vehicle and the foam appears to be effective, safe, and well-tolerated. This may provide a good alternative for rosacea sufferers looking to avoid oral medications.

Sebaceous Cyst Removal

In this video, Dr. Nakatsui removes a sebaceous cyst from our patient’s scalp. The cyst came out whole, cleanly, and intact! Many times, if a cyst becomes infected it can adhere to the surrounding tissue making extraction trickier as it can fragment and stick to surrounding tissue.

Overall a great little surgery which great end results!

Sebaceous Cyst Removal

September 28th, 2019 | Treatments, Video

In this video Dr. Nakatsui is removing two sebaceous cysts located on the scalp. Our lovely patient graciously allowed us to film the procedure and we thank you for that!

In this video the two cysts come out relatively easily and intact as a whole. There will be several instances where we are not so lucky and the cyst may fragment or it may adhere to surrounding tissue more making extraction more difficult.

Sebaceous cysts may occur anywhere on the body at any time of our lives. They are generally not life-threatening, but when concerned about any lesion, it is always best to seek medical attention. Many patients with sebaceous cysts may leave them as they generally will not bother them. From time to time, cysts may become inflamed, painful, produce discharge, or become infected. If a cyst has had repeated infection/inflammation or self attempts to extract the contents by squeezing the cyst, this is when the cyst wall may become more adherent to surrounding tissue making it difficult to extract.

Should a cyst develop, it is advisable not to perform self surgery as any tools used to puncture the skin or extract the cyst may push unwanted bacteria into the site causing an infection. Not only that, if the wound site is not properly closed or should become infected, when it fully heals you may be left with an unwanted scar which may be difficult to treat in the future.

WARNING: GRAPHIC CONTENT

VIEWER DISCRETION IS ADVISED

In this video, we are seeing what Dr. Nakatsui sees when performing surgery. This surgery is the removal of a lipoma, a benign fatty growth that can occur anywhere on the body. Many are small, but lipomas can grow very large in size. Dr. Nakatsui makes a small excision where the lipoma is located. This particular lipoma came out relatively easily, but keep in mind that not all lipomas will be this easily extracted. To close the surgery site, Dr. Nakatsui uses a subcuticular running stitch.

Ohhhh the Weather outside is frightful, but the fire is so delightful, and since we have places to go, I want my skin to look aglow look aglow!

Yes!  The Merriest time of year is upon us.  Family dinners, work Christmas parties, and having friends over for the evening is all part of this festive season!  The truth of the matter is, is that we want to look our absolute best at these events.  We want to have smooth skin, amazing eyes, and a body that fits perfectly into that cocktail dress.  How we look at the end of the evening is a different story!

At the Groot DermaSurgery Centre, we offer various treatments that help fine-tune and tweak the little things.  Don’t like the wrinkles around your eyes whenever you smile?  Botox Cosmetic can be used to help treat crows feet so that when you’re smiling for photos, the wrinkles around your eyes will not steal the show.  What about your skin?  You want it to be flawless in those selfies.  HydraFacial MD can help cleanse the skin and reintroduce important compounds back into the skin that have been lost due to the harsh summer weather.

Eyelash extensions and mascara are great ways to lengthen your lashes for a short, temporary period. A proven way to lengthen and thicken your own lashes is through the use of Latisse. Latisse is the first and only FDA-approved, science-based treatment that has been clinically proven to actually grow noticeably fuller, longer, and darker lashes. Latisse ® works by lengthening the growth phase of the hair follicles of each eyelash. By prolonging the growth phase of the eyelash follicle, the eyelash hair grows longer, fuller, and thicker

How about the double chin that is always there whenever you look back at photos?  Belkyra injections and ThermiTight can help eliminate fat and improve on the appearance of double chins!  That cocktail dress fit last month, but it’s a little bit more snug this month, and you’re also not happy about that little bulge?  CoolSculpting is a non-surgical and non-invasive way to reduce fat with minimal downtime.

The best time to have these treatments performed is at least 2 weeks prior to your engagement. Botox can often take 10-14 days to take full effect.  Fillers and any injections can cause swelling which can take up to 7 days to resolve. At the Groot DermaSurgery Centre, all Botox, Filler, and Belkyra injections are performed by Dr. Nakatsui, a board certified Dermatologist. All other procedures are performed by registered nurses trained extensively in all manners of dermatology.

Let’s talk about scars

July 27th, 2018 | General Info, Treatments, Uncategorized

Scar treatmentScar treatment can be challenging and requires a multi-dimensional approach in order to be successful. A scar can be the centre of attention but can also be a source of annoyance. For some individuals, scars are a part of who they are, whereas for others they are intrusive and cause insecurity. They can be caused by a variety of reasons such as acne scarring, trauma, and surgery.

Some individuals have thickened keloid scarring, where a scar becomes raised and discoloured. In order to treat the thickened aspect of the scar, many times cortisone injections will be utilized to reduce the thickness of the scar.

On the opposite end of thickened scars, people may get pitted scars where the skin sinks in various regions. These types of pitted scars can be tricky to treat; however, with improvement in technologies and techniques, pitted scars can often be improved. In some cases, Dr. Nakatsui will use an ablative laser to smooth out the skin and reduce the harsh edges that cause shadowing.

A new approach is to promote collagen re-growth in the region.  Through the use of a technology called Percutaneous Collagen Induction (PCI), many tiny needles create tiny holes in the skin. The creation of these tiny holes stimulates collagen formation to help fill in the areas of depressed scarring.

Scar treatment also encompasses hyperpigmentation (darker pigmentation), hypopigmentation (lighter pigmentation), and erythema (redness).  Hyperpigmentation and erythema can be treated fairly successfully with various lasers. Unfortunately, hypopigmented scars are more difficult to treat and currently there is no reliable solution to permanently bring colour back into a scar once the colour is gone.

Certain scars due to trauma are very visible to the human eye because the lines of the scar are fairly linear. Surgically removing the scar and carefully using various closure techniques to break up the linear scar can greatly reduce the appearance of a scar.

Scarring can impact an individual’s self-confidence. With current technology, scars can be improved. For those with scars that affect their self esteem and their daily living, don’t get discouraged as there are many great options to help treat your scars.

For more information regarding the various treatment options for scars please visit: scar revision for more information or you can contact the Groot DermaSurgery Centre at (780)482-1414.

What can be done about beards?

September 15th, 2017 | General Info, Hair Transplant, Treatments

Tormund Giantsbane - Game of Thrones

In today’s society having a full and lush beard is becoming more and more of a fashion statement for men.  While some men are born with thick facial manes, others require treatment to aid in thickening up their beards.

Not all men are lucky like actor Kristofer Hivju (photo), more popularly known as Tormund from the HBO hit show, Game of Thrones (most of the staff at the Groot DermaSurgery Centre are very big fans of Game of Thrones).

Dr. Thomas Nakatsui, a board certified Dermatologist and hair transplant specialists, has been performing hair transplants for over 18 years.  The exact reasons why men come in for a beard transplant vary.

Some men want to increase thickness, while some men have suffered some form of trauma that caused an area to scar, therefore leaving a portion of facial hair missing.

As with any hair transplant, artistry plays a major role in how the end result turns out.  Hair transplants need to look natural and undetectable in appearance.  Old forms of hair transplants, “plug transplants”, have yielded unnatural and very detectable results.

Having undetectable results is even more paramount whenever dealing with the face as it is always front and center.  Any irregularities can be easily spotted.

To address this issue, Dr. Nakatsui follows the patient’s natural flow, angle, and direction of their beard.  If hairs are placed in any strange angle, it will become very apparent that a hair transplant was performed once the hairs have grown in.

The size of the graft is important as well.  A graft is a natural grouping of hairs that come in 1, 2, or 3 hair bundles.  Dr. Nakatsui typically utilizes the grafts with one hair when it comes to beard hairs.

Beard hair transplants can be a great tool to help thicken up a beard.  One thing to consider before having a beard hair transplant is that the donor hair, which is usually taken from the back of the scalp, may have different texture and/or colour than the recipient bearded area.

Once the transplanted hairs are placed, it will hold the same characteristics as the area where it was taken from.  If there is a significant difference between the donor hair at the back of your scalp and your beard hair, it can potentially become visible and unnatural.

The beautiful thing with a beard hair transplant if it is done correctly is that it now gives the individual the choice whether or not they want to grow a beard or be clean shaven.

Because of the incredibly small incisions that Dr. Nakatsui makes during the procedure, a patient can be cleanly shaven and have no trace of scarring to the area.

Removing The Regretted Tattoo

August 31st, 2017 | Conditions, General Info, Treatments

We’ve heard it all before: a woman falls deeply in love with her boyfriend, Kevin, and decides to have his name tattooed across her arm only to become heartbroken the following week.  She is now stuck with this unwanted tattoo across her arm.

Years ago, she only had a few options: live with the regret and have his name forever inked on her skin, or try to cover it up with a second tattoo on top of her current tattoo.  Various creams have been suggested; however, many of them were either ineffective or, worse yet, caused scarring due to the strong nature of the creams.

The newest and most advanced technique for removing unwanted tattoos is with laser therapy.  At the Groot DermaSurgery Centre, we use the Q-switch Alexandrite laser.  Various lasers are used for various conditions such as excessive hair, mole removal, or unwanted blood vessels.  Each one of these conditions will require a different wavelength light.

Specific to tattoos, the laser penetrates the skin at a specific wavelength with the goal of breaking pigmented particles.  Your body’s immune system then removes these pigmented particles over time.  After a few treatments, the tattoo can potentially be completely erased from a person’s skin.

As with any procedure, especially those that involve lasers, it is always important to be aware of the risks and precautions surrounding the treatment.  Regarding laser therapies for tattoo removal, one potential risk is hypopigmentation, or the loss of colour, of the skin.   Or conversely, a person may get hyperpigmentation, or the increase in pigmentation in the skin.  The laser used in tattoos cannot distinguish between naturally dark pigmented skin and a tattoo, therefore, when the tattoo is being removed, surrounding skin can potentially be affected as well.

At the Groot DermaSurgery when consulting an individual about tattoo removal, we highly suggest that the individual not have any tan in their skin before and after treatment for these exact reasons. As always, speak to a certified medical professional when dealing with any treatment. If you have any questions or you would like more information, please do not hesitate to contact the Groot DermaSurgery Centre and book a complimentary consultation for your specific needs or please visit this link.

*At the Groot DermaSurgery Centre we love Kevins and even employ a Kevin at our facility!  We only used your name as an example*

What are the dangers of cheap or unapproved Botox and Dermal Fillers?

August 23rd, 2017 | General Info, Treatments

15_06_24_bargain_botox_0

When it comes to Fillers and Botox®, it is important that you know what the product is and who is doing the treatment and to be wary of discount botox or discount filler injections as the long term results can be more costly. Dr. Nakatsui has been asked to consult on many patients who have had problems with their fillers and Botox.

Having an approved physician that has been properly trained in the use of Botox or dermal fillers is very important. Not only is the cosmetic end result important, but also having the knowledge and training should there be any complications.  As with any elective procedure, it is important to know who will be doing the procedure and what types of products they will be using.

As we age, our natural fat pads and bone density decrease.  Excessive motion in the face can cause wrinkling around the eyes, forehead, and mouth.  Through the use of Botox® Cosmetic and Dermal Fillers such as Restylane and JUVÉDERM®, patients can achieve amazing and wonderful results that can help reduce fine lines, smooth out sunken areas of the face such as the cheeks and nasolabial folds, or to enhance the lips.

Dr. Thomas Nakatsui, a board certified Dermatologist, has used and implemented approved Botox® Cosmetic and Dermal Fillers such as Restylane and JUVÉDERM® with great success for over 15 years. Using a variety of techniques, Dr. Nakatsui is able to help patients rejuvenate their face and help patients achieve a more youthful look.

If you have any questions, please feel free to visit our website at nakatsuiderm.com or do not hesitate to call our office at (780)482-1414 and any of our friendly staff will be able to assist you. Thank you and have a great day!

The Dreaded Man-breast

May 2nd, 2017 | Conditions, Treatments

You’re working out, eating well, and simply doing all of the right things.  You look in the mirror and see your abs toning up; your biceps getting bigger.  Things are going great… except, your just not satisfied with your chest because it still has some fatty deposits that make it appear like breasts.  Fatty deposits on the chest can affect many males and can be difficult to treat on your own no matter how strict you are with diet and exercise.

In comes CoolSculpting, a non-invasive technology that freezes away stubborn fat.  An applicator is placed on the problem area, in this case the chest, and gently targets and cools unwanted fat cells in the selected area.  Millions of CoolSculpting Procedures have been performed worldwide with great success!  The Groot DermaSurgery Centre was one of the first clinics to offer CoolSculpting in the city!

CoolSculpting can be effective in reducing the fat in the chest region of a man.  However, not all man breasts are simply fatty tissue.  A condition known as gynecomastia is when there is an excess of male breast tissue.  In this situation, CoolSculpting may not be an ideal treatment.  It is important to have a consultation for a proper assessment to determine if you are a candidate for CoolSculpting.

If you would like to book a complimentary consultation please do not hesitate to contact our clinic at (780) 482 – 1414.

For your convenience, Botox and filler appointments, as well as Coolsculpting, Thermitight, and HydraFacial MD appointments can be booked directly online or by calling the office directly at 780-482-1414. There is a $100.00 refundable deposit required to reserve an appointment using the online booking service (please note there is a $20.00 administrative fee deducted from this deposit). If you cannot find a convenient time using the online scheduler, please call as there may be additional appointment times available that are not visible online.

Dr. Nakatsui Double Diamond award recipient from AllerganDr. Nakatsui is one of a small number of solo physician injectors to have received Diamond and Double Diamond status from Allergan, the makers of Botox and Juvederm.

We are proud of having received this recognition but it is not essential to go to a Double Diamond provider to be certain you have an injector with expertise. When considering an injector, look for a practice that uses Botox and fillers every day. Ask about experience and training with Botox and fillers. Find out who your injector is–injectors are usually physicians, but can be nurses or pharmacists. At the Groot DermaSurgery Centre, Dr. Nakatsui personally performs all Botox and filler injections.

Be aware that Double Diamond status can be achieved by multi-physician groups and multi-location practices where Botox is distributed to multiple physicians and injectors. Therefore, not all Silver, Gold, Platinum, Diamond, and Double Diamond providers actually use the same amount of Botox or fillers.

What Do You Do When Antiperspirants Don’t Work for Hyperhidrosis?

August 24th, 2016 | Conditions, Treatments

HyperhidrosisDespite the fact that sweating serves the very important function of cooling the body it is considered to be unacceptable in certain social settings. For example a sweaty palm during a handshake is often frowned upon whereas sweaty palms while exercising falls within most people’s tolerance level. Sweat marks under the armpits while dining at a restaurant is socially undesirable whereas arm pit sweat while working in intense heat would be considered understandable.

For those socially awkward situations antiperspirants work for most individuals to control underarm sweating. However there is a condition known as hyperhidrosis where excessive sweating in any circumstance is a problem. The problem may be limited to the armpits, but often the palms of the hands and soles of the feet produce excessive amounts of sweat as well. Antiperspirants fail to work in individuals with hyperhidrosis.

There is no cure for hyperhidrosis. The good news is that there are controls. Sweat inhibiting injections of Botox have proven to markedly reduce sweating for anywhere between 3 to 16 months (average of 6 months) after 1 or 2 treatments.  The injections have been approved by Health Canada for the treatment of excess underarm sweating.

Topical applications of aluminum chloride hexahydrate, may provide satisfactory sweat control for sufferers of hyperhidrosis.  In Canada, a 20% aluminum chloride hexahydrate alcohol solution is available without prescription under the name of Drysol.

Other options may include internal medications (such as acetycholine), electrophoreses, the Drionic machine, or dilute formalin (for sweaty feet).  In very rare cases surgery is necessary to remove the sweat glands or sever the nerve responsible for their stimulation.

If you would like more information, please do not hesitate to contact our clinic at (780) 482-1414.  Thank you!

Hope for Ugly Toenails

June 28th, 2016 | Conditions, Treatments

wedge-sandals

Are you embarrassed to wear sandals because of your ugly toe nails – toe nails that are yellow, brittle and/or scaly? If so, it is possible that you have fungus of the toe nails (onychomycosis) – a condition that can be treated.

Two options are available: medication and laser treatments. Lamisil, Soranox or Nizoral are examples of prescription anti-fungal medications that are taken by mouth for a period of 3 to 6 months. They work from the inside out (systemically) with about a 70% success rate.  A new medication to the market called Jublia  is a topical ointment that is applied directly onto the problematic nail.

Lasers work from the outside in. Two treatments and the fungus is eradicated in 70% of cases. The success rate depends on how long you have had the fungus. If it has been left untreated for over a year then a combination of medication and laser treatments is usually required.

If you are hiding your toes, call us at (780) 482-1414 to find out if one of these treatment options might work for you. Then go out and buy a pair of those cute little sandals that you have been coveting.

Fillers and Botox for cosmetic reasons: Truths and Misunderstandings

June 28th, 2016 | Treatments

Portrait of a young woman has a question is asking herself something. Question mark over her head.

As we age, certain aspects of our appearance begin to bother us when we look in the mirror.  What we see in the mirror may not reflect what we feel on the inside.  The vivaciousness and energy that we carry may not be indicative of the bags under our eyes.

As we age, certain aspects of our appearance begin to bother us when we look in the mirror.  What we see in the mirror may not reflect what we feel on the inside.  The vivaciousness and energy that we carry may not be indicative of the bags under our eyes.

Botox cosmetic and dermal fillers such as Juvederm and Restylane have been around for many years.  They each have a specific role to play when dealing with facial rejuvenation. Botox and fillers can often become mixed up in their perceived usage.  Many times when a celebrity with large lips is seen on TV, one of the common phrases is that she has too much Botox in her lips.  In all likelihood, that celebrity had a dermal filler treatment to enhance her lips.

Botox (Onabotulinumtoxin A) is a substance that blocks localized nerve activity in the muscle.  This causes a temporary cessation of muscle movement.  This sounds frightening, but when Botox is injected into specific muscles in can produce a variety of fantastic results that will help reduce wrinkles.

Botox can be used for a variety of other medical conditions.  Chronic Migraine sufferers will use Botox to help reduce tension headaches.   Botox can also be used for individuals suffering from excessive sweating (hyperhidrosis).  Onabotulinumtoxin A will temporarily block the secretion of the chemical that is responsible for turning on the body’s sweat gland.

When Botox is injected into very specific muscles in the face, it can reduce the overall wrinkling of certain portions of the face.  The area in between the eye brows (glabella) can become furrowed, therefore causing an angry look.  The lines on the forehead (frontalis) can become very wrinkly with continuous movement thus causing a face to appear much older than it actually is!  Crows feet on the side of the eyes can become wrinkled through years of smiling and laughing and many individuals like that look.  For those that do not appreciate that look, Dr. Nakatsui can help.

Dermal fillers such as Juvederm and Restylane are forms of hyaluronic acid, a naturally occurring sugar found in the body.  Properly placing these dermal fillers, Dr. Nakatsui is able to restore the fullness that certain portions of your face once had but lost due to the natural aging process or weight loss.

These fillers can also be used in accentuating the great features that you already have, such as the lips.  By placing filler in the lip line to emphasize your lips, you can get a very big impact without having the fish lip look.  Dr. Nakatsui’s goal is to make subtle changes to make the areas appear natural as if they have always been that way.

The objective of Dr. Nakatsui and the Nakatsui DermaSurgery Centre is to enhance the great features that you already have.  Through the use of Botox or Juvederm, the goal is not to overhaul and completely change your face, but rather take the great features you have and make them shine!  If you feel that your looks do not reflect how old you feel, there are treatments that can help.

Through the advancement of laser technology, fine lines, wrinkles, and scars can become a thing of the past!  Fractional lasers such as Fraxel and Pearl Fractional place tiny holes deep in the skin selectively removing columns of damaged tissue leaving the surrounding skin intact. Patients see an improvement in photo damage, tone, and texture, as well as a noticeably smoother  and brighter complexion. The Pearl Fractional is often combined with the Pearl to achieve maximum benefit (Pearl Fusion). The photo  shows one of our patients after one treatment with Pearl Fusion. Additional benefit can be seen with more treatments.
